Online Registration

Licensed pharmacists, physician assistants, certified registered nurse practitioners, certified nurse midwives, and practitioners must be licensed in the state of Alabama to apply to access the controlled substance database. If you do not hold an Alabama license, visit Related Links to find and apply with the appropriate licensing board.

The controlled substance database is a web-based tool developed to assist licensed prescribers, dispensers, law enforcement personnel, and regulatory boards. Licensed prescribers, dispensers, and law enforcement personnel must apply to access the Prescription Drug Monitoring Program's (PDMP) website.

To obtain instructions on accessing the controlled substance database, visit the Prescription Drug Monitoring Program's (PDMP) new website or select the appropriate training guide below.

If you need additional assistance, please contact the PDMP Help Desk at 1-855-925-4767 (option 1 for questions about a patient's history, option 2 for questions about reporting data).

Visit the PDMP website to request access to the PDMP controlled substance database.

Visit Educational Materials for more information on prescription drug misuse and abuse.
