Cribs for Kids® - Alabama
Alabama has partnered with the national Cribs for Kids® Program and is dedicated to "Helping every baby sleep safer." Funding from the federal Maternal and Child Health Services Title V Block Grant has allowed Alabama to purchase a limited number of pack-n-plays for families who are in need of a safe sleep environment for their infants under one year of age. The pack-n-plays are provided through referrals from partner agencies. The required forms are below.

The following packet of 3 forms are required to be filled out completely when referring a client to the Cribs for Kids® - Alabama program.
Completed referral forms and questions regarding the program can be emailed to: SafeSleep@adph.state.al.us.
