Program Overview

About half of all U.S. adults have at least one chronic disease: 75 million have high blood pressure, 84 million have pre-diabetes, and 30.3 million individuals have diabetes. This program will discuss the role that professionals play in team-based care as well as their effectiveness in reducing A1C’s, diabetes complications, and better improvement of blood pressure.
